Things To Ask Your Insurance Provider

If you have health insurance be sure to contact your insurance company to verify coverage for counseling services. Some companies may cover the services in full or partially.

1) Do I have mental health benefits?
2) Is pre-authorization required?
3) Is there a limit on number of benefits?
4) Do I have a deductible or co-pay?

Cancellation Policy

We require you to provide a 24 hours notice to cancel your visit.  If you do not cancel or show up for your visit 24 hours prior, there will be a $75 charge at the time of your scheduled visit.  We send reminder texts and emails. We also ask you to put the appointment in your calendar.

Practice Policies

Insurance verification: While we do our best to verify insurance, the information given to us by the insurance company may not always accurately reflect a patient’s policy and coverage. Patients are responsible for understanding their plan and coverage.
 
Phone/email contact: Please contact our office during working hours Mon-Fri  9. 00-5.00PM. Our office phone is usually answered during office hours.Please allow 24 hrs. for provider to respond to email messages.
 
Payment: Payment is due at the time of service. The client is responsible for all balances after insurance payment is received.  Accounts not paid will be referred to a collection agency and incur a 40% service fee. Clients with accounts in arrears will be terminated from the practice.
 
Cancellations/missed appointments: Cancellations require a 24-hour notice. Clients missing their first psychiatric appointment will forfeit the $100 deposit. Missing/late cancellation for a follow-up appointment or no show will incur a $75 fee.
 
Prescription refills: Please call one week in advance for medication refills. Prescription requests following a missed appointment will incur a $35 fee in addition to the fee for the missed appointment and will be charged to credit card on file. CONTROLLED MEDICAITONS (Xanax, Valium, Klonopin, Ativan, etc.) originally prescribed by other subscribers will be evaluated before being prescribed/refilled by the provider.
 
Requests for paperwork: Fees for paperwork requests vary depending on the records requested.
 
Disability: I DO NOT perform disability assessments or complete paperwork for disability claims. All other paperwork requested must be discussed during office visit and may be at a fee.
 
HIPAA Notice of Policies and Practices: The Health Insurance Portability and Accountability Act state law require that I protect the privacy of your personal information. A patient must sign a consent for their medical record to be released to other provider (s).
